Types of massage (bodywork) therapy - therapeutic massage focuses on specific points of pain and modalities to relieve that pain or improve function; wellness massage for relaxation and general health maintenance, oncology massage is geared toward relaxation and pain relief for clients with a history of cancer or currently receiving treatment for cancer; and manual lymphatic drainage therapy is geared for individuals living with chronic lymphedema although other conditions are well supported with this bodywork. In general, all of these modalities help clients manage stress, tension, and pain, thereby improving their qualify of life. Hydration is always important for receiving maximum benefits as a healthy fluid balance will promote joint function, flush toxins, and decrease tension in the muscle and fascia.
Confidentiality of your personal goals and health records is assured; however, in the event Spark of Life is able to bill your health insurance the claims payer will typically require chart notes to document medical necessity and they may require a prescription to be on file with your massage therapist.
Clients are always draped for privacy and only the areas being worked are undraped and the blankets are secured. Clients will have time to disrobe to their comfort level and place themselves under the covers on the massage table, before your LMT enters the studio. Note that a low pressure relaxation massage can be administered without disrobing with use of a satin massage cloth. Please inquire with your massage therapist for more information.
A waiver and release must be signed before treatment begins. At the start of each new session your massage therapist will inquire about current health conditions, provide a covid screening and day of liability waiver.

Cancellation - A 24-hour notice is required for cancellation of an appointment, or you will be charged a $40.00 fee for the first missed appointment. Payment is due before your next appointment.
General conduct - Appointment times are as scheduled and cannot be extended. Management reserves the right to terminate services immediately for harmful acts of physical, verbal, and written or other forms of harassment.
During session, clients are encouraged to quiet their minds, reduce conversation to what specifically is helping them during treatment. Refraining from talking helps you become more attuned to the physical sensations and feedback from your body.
Sickness - Massage (bodywork) is not appropriate care for infectious or contagious diseases and other serious uncontrolled health conditions. Please cancel your appointment as soon as you are aware of such an illness. If it is within the 24-hour notice period, the cancellation fee may be waived.
Financial responsibility - massage therapy is billed at $2.00 per minute. If insurance can be billed, co-pays and/or co-insurance will be billed to you after the appointment by the billing service and your insurance is billed for the service in 15-minute increments under CPT codes 97124 or 97140. We accept payment from your insurance company for covered services. If the insurance company denies payment or makes partial payment, you are responsible for the balance, deductibles and co-pays. A signature is required affirming financial responsibility.
Release of medical records - Your signed acknowledgement allows this office to release chart notes on file in this office, for the purpose of processing claims, to the following: your attorney, the healthcare providers attending to this condition, and the insurance case managers.
